Processing of personal data – Patient information notice

Who processes your personal data?

The Brussels Skin Center SRL, whose registered office is at 165 E Avenue Winston Churchill, Mailbox 70, 1180 Brussels (Belgium), registered at the Banque-Carrefour des Entreprises under number 0739.788.613 (the ‘Center’), collects and processes certain of your personal information.

Categories of personal data processed

Your personal data includes your surname, given name, civil status, language, date and place of birth, mobile telephone number and email address, the care provider looking after you at the Center, the date and time of your appointment, your health insurance provider, and your ID in the Center’s management system. In addition to this administrative information, your medical record is also processed by the Center. Any photographs taken as part of your care or consultation form part of your medical record and are necessary for the diagnosis and monitoring of health conditions. Your entire medical record is covered by professional confidentiality. Medical information of a personal nature is processed under the supervision and responsibility of a doctor.

Purpose of processing

Your personal information is processed for the following purposes: to provide your medical follow-up, to create and keep your medical record up to date, to invoice your treatment, to prescribe medication and care, to request tests and to manage continuity of care with other care providers, where applicable.

Legal basis of processing

The legal basis of the processing of non-medical information is necessity in order to draw up the healthcare provision contract and for the legitimate interests of the Center (for contacting the patient). The legal basis of the processing of health-related information is necessity under the contract entered into with a health professional, and subject to professional confidentiality, or under his/her responsibility.

Transfer of personal information

The individuals who have access to this information are the Center employees and your care providers. You acknowledge that your personal information is also accessible to the Center’s technical providers, exclusively for the purposes mentioned above, and with regard to which the Center has taken the appropriate contractual measures to protect your personal information, in accordance with the applicable legislation.

Retention period

In accordance with current legal requirements, the following retention periods apply to your personal information: (i) your health data will be kept for a minimum of 30 years after your last contact with the Center, (ii) your billing data such as accounting documents will be kept for 15 years, and (iii) your other personal data will be kept for 5 years after their last use, except where the Center has a specific, legitimate interest in keeping your personal information for a longer period (for example, insofar as the information may be required in order to fulfil a legal obligation). Once the above-mentioned retention period has elapsed, the personal data will be deleted and destroyed within 12 months.

Your rights

You have the right to access your personal information, as collected and processed by the Center, and to request the correction or removal (where applicable) of this information if it should be incorrect or unnecessary. You also have the right to restrict processing or object to it. To exercise your rights, simply send a request to the following email address: privacy@brussels-skin-center.be, or to the postal address given above with a copy of a document proving that you are the person to whom this personal information relates.
